My wife had Symphonix Health Insurance as a supplement to her medicare. it was ok, except when I changed jobs and put her on my employers health insurance. When we tried to cancel her Symphonix, the person we called said it was no problem and we thought nothing of it. A year later, we got a bill from them in the mail. They had charged her a premium, even though we had cancelled. We called again, and they said they would take care of it, and we left it alone. We got another bill. When we called this time, they said we had to fill out a form and send it back to cancel. We got the form, and sent it back, but they said it was received late, and we still had to pay the premiums we had incurred over the past year that we had not used their plan. We filed an appeal, and they reviewed it, and declined it. They said we still have to pay, or they will send it to collections.
